Bronx Health Network, Inc. Seeks Community Health Worker Supervisor

General Job Statement: The Community Health Worker (CHW) Supervisor reports to the Director of Programs and will primarily be managing the CHW program to ensure that the program is working efficiently.  The supervisor will also work out in the community with specific target populations.  The CHW supervisor will work with primary care teams of our health centers and other agencies to improve patient care and outcomes.

Key Responsibilities:

Administrative:

  • Identify barriers and facilitators to enrollment assistance and revise the program plan as appropriate.
  • Collaborate with other health centers and providers to ensure that outreach and enrollment assistance activities are coordinated.
  • Develop and maintain partnerships with community partners.
  • Direct and facilitate interactions with community partners.
  • Identify and compile a list of health events that BCHN can participate.
  • Maintain monthly calendar of health events.
  • Assign health events to patient navigators and CHWs.
  • Assist in preparing health educational materials.
  • Maintain inventory of health education materials.
  • Represent BCHN at community meetings and present BCHN’s CHW program including health insurance exchange program.
  • Collaborate with Data Manager to provide input on the development of tracking and reporting tool for the CHW program.
  • Assist Director for Programs in evaluating progress of CHW program and revising strategies as appropriate.
  • Ensure that CHW program reports are completed and submitted on a timely basis.
  • Successfully complete all trainings required and applicable federal and/or state consumer assistance trainings.
  • Supervise the CHWs.
  • Take on special projects as assigned.

Outreach

  • Perform outreach and enrollment assistance activities in a culturally and linguistically appropriate manner.
  • Conduct public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace.
  • Help individuals understand and access affordability options.
  • Identify health care needs and strengths of the targeted community.
  • Identify community members without a health insurance or PCP and connect them to BCHN health centers.
  • Refer community members to needed social support services.
  • Help community members develop health management plans and goals and follow up to ensure they have completed required or recommended actions and track progress.
  • Motivate community members to be active, engaged participants in their health.
  • Coordinate and participate in health screening, health fairs, workshops and other health events for the targeted community. Advise community members on issues related to diagnostic screenings; self-management and improving general health.
  • Interpret, translate, or provide cultural mediation related to health services or information.

Qualifications:

A minimum of a Bachelor’s Degree from an accredited college in public health, health education correlated field.  Master’s degree in a related field – a plus.

A minimum of 2 years of health care and supervisory experience.  Must have extensive knowledge of the health care system and the different organizations that provide social and support services to their target population. Excellent organizational skills to plan and prioritize work. Must have strong interpersonal skills in networking with other community stakeholders. Proficiency in Microsoft Word, Excel, Power Point, spreadsheets and database.

Bilingual:  English/Spanish a plus.

Excellent communication, customer service and interpersonal skills.  Proficiency in engaging patients.

Excellent presentation skills and proficiency in engaging an audience.

New York State Association for Affordable Housing Seeks Policy Director

Description:
The New York State Association for Affordable Housing (NYSAFAH) is the trade association for New York’s affordable housing industry statewide. Our 375 members include for-profit and nonprofit developers, lenders, investors, attorneys, architects and others active in the financing, construction, and operation of affordable housing. Together, NYSAFAH’s members are responsible for nearly all of the housing built in New York State with federal, state or local subsidies.

Summary:

NYSAFAH seeks a Policy Director to coordinate policy positions on a local, state and federal level and to develop working groups to help establish policy priorities. The Policy Director will review public and academic reports, white papers, articles and other publications relevant to housing. He will work with NYSAFAH’s staff and board to develop policy-related programming as well as to communicate policy and advocacy positions to the public, press and membership at large. This is an excellent opportunity for an experienced housing policy advocate that enjoys working in a fast-paced, team-centered environment.

Responsibilities:
• Work with NYSAFAH staff and board to develop policy-related programming
• Represent NYSAFAH at City, State, and Federal housing-related meetings, hearings, and events
• Work with NYSAFAH lobbyists to draft and submit NYSAFAH public testimony
• Testify at public hearings
• Mobilize membership in response to proposed legislation or regulatory changes
• Respond to meeting and information requests from public officials
• Coordinate NYSAFAH policy and advocacy campaigns with NYSAFAH committees and City and State lobbyists
• Coordinate policy and advocacy campaigns with coalition partners
• Organize affordable housing informational briefings

Qualifications:
• Bachelor’s degree required; preferred areas of study –  policy, government relations, communications, real estate or urban planning. Masters preferred (similar studies).

CAAAV Organizing Asian Communities of New York City Seeks Public Housing Organizer (Chinese-speaking)

  • Building the leadership of Chinese tenants living in NYCHA through know your rights trainings, political education, and skills trainings.
  • Recruiting, managing, and training bilingual volunteers and interns to conduct outreach.
  • Building relationships with community organizations and Queensbridge’s Tenant Association to engage Asian tenants in larger multiracial organizing efforts.
  • Conducting consistent outreach/doorknocking with Chinese tenants for meetings and actions.
  • Holding monthly membership meetings for Mandarin and Cantonese speaking tenants informing and engaging their ideas on CAAAV’s strategy/tactics.
  • Holding weekly office hours at Queensbridge assisting tenants with their needs regarding interpretation/translation of key NYCHA documents as well as navigating NYCHA’s Customer Contact Center (CCC) for repair issues.
  • Coordinate broader NYCHA campaign goals/strategy with Bengali and Korean tenant organizers and respective members through weekly meetings and other systems of communication.

  • Demonstrated ability to develop and supervise staff.
  • Ability to design, lead, run workshops and trainings for members that can be catered to members with different language abilities and backgrounds.
  • Ability to meet deadlines and manage multiple projects at the same time.
  • Ability to work flexible hours, including evenings and weekends.

The ASPCA Community Advocate engages in grassroots efforts to reduce the number of companion animals surrendered to shelters and improve the welfare of pets and the people who care for them. The Community Advocate will accomplish this by building trusted partnerships with community residents in New York City’s most underserved neighborhoods, and connecting them to ASPCA services and other resources that increase pet health and spay/neuter rates.

The Community Advocate schedule is Tuesday-Saturday and generally requires an early morning start once a week (6:00 AM/7:00AM) and/or full weekend shifts approximately twice a year.

Responsibilities

  • Build relationships with community residents to promote and connect with people in need of ASPCA CARES program services, including several hours of weekly door-to-door outreach in a designated area of the New York City
  • Plan, promote and execute events to connect people with the most at-risk pets in New York City to the services that the ASPCA CARES program has to offer
  • Share information regarding the benefits of spay/neuter
  • Schedule spay/neuter appointments & follow up to ensure appointment compliance
  • Meet and greet clients at clinic prior to intake
  • Transport pets to clinics as needed
