Bronx Health Network, Inc. Seeks Community Health Worker Supervisor

General Job Statement: The Community Health Worker (CHW) Supervisor reports to the Director of Programs and will primarily be managing the CHW program to ensure that the program is working efficiently.  The supervisor will also work out in the community with specific target populations.  The CHW supervisor will work with primary care teams of our health centers and other agencies to improve patient care and outcomes.

Key Responsibilities:

Administrative:

  • Identify barriers and facilitators to enrollment assistance and revise the program plan as appropriate.
  • Collaborate with other health centers and providers to ensure that outreach and enrollment assistance activities are coordinated.
  • Develop and maintain partnerships with community partners.
  • Direct and facilitate interactions with community partners.
  • Identify and compile a list of health events that BCHN can participate.
  • Maintain monthly calendar of health events.
  • Assign health events to patient navigators and CHWs.
  • Assist in preparing health educational materials.
  • Maintain inventory of health education materials.
  • Represent BCHN at community meetings and present BCHN’s CHW program including health insurance exchange program.
  • Collaborate with Data Manager to provide input on the development of tracking and reporting tool for the CHW program.
  • Assist Director for Programs in evaluating progress of CHW program and revising strategies as appropriate.
  • Ensure that CHW program reports are completed and submitted on a timely basis.
  • Successfully complete all trainings required and applicable federal and/or state consumer assistance trainings.
  • Supervise the CHWs.
  • Take on special projects as assigned.

Outreach

  • Perform outreach and enrollment assistance activities in a culturally and linguistically appropriate manner.
  • Conduct public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace.
  • Help individuals understand and access affordability options.
  • Identify health care needs and strengths of the targeted community.
  • Identify community members without a health insurance or PCP and connect them to BCHN health centers.
  • Refer community members to needed social support services.
  • Help community members develop health management plans and goals and follow up to ensure they have completed required or recommended actions and track progress.
  • Motivate community members to be active, engaged participants in their health.
  • Coordinate and participate in health screening, health fairs, workshops and other health events for the targeted community. Advise community members on issues related to diagnostic screenings; self-management and improving general health.
  • Interpret, translate, or provide cultural mediation related to health services or information.

Qualifications:

A minimum of a Bachelor’s Degree from an accredited college in public health, health education correlated field.  Master’s degree in a related field – a plus.

A minimum of 2 years of health care and supervisory experience.  Must have extensive knowledge of the health care system and the different organizations that provide social and support services to their target population. Excellent organizational skills to plan and prioritize work. Must have strong interpersonal skills in networking with other community stakeholders. Proficiency in Microsoft Word, Excel, Power Point, spreadsheets and database.

Bilingual:  English/Spanish a plus.

Excellent communication, customer service and interpersonal skills.  Proficiency in engaging patients.

Excellent presentation skills and proficiency in engaging an audience.
